Both settings live in the desktop app under Overlays → Hotkeys, below the hotkey list.
Scroll triggers
Two triggers act while Path of Exile 2 is active. Each has its own on/off switch, a Modifier, and an action for Scroll up and Scroll down.

- Modifiers: Ctrl, Alt (either), Left Alt, Shift, Ctrl+Shift, Ctrl+Alt or Shift+Alt.
- Actions: Left click, Previous stash tab, Next stash tab, or Nothing.
- Ctrl and Alt must match exactly; Shift is ignored unless you pick it.
- If both triggers use the same modifier, only the click trigger fires — the page warns you.
Mouse-button blocking (optional, Windows)
A hotkey can be bound to Mouse 3, 4 or 5. Normally the game receives that press as well. Turn on Block Bound Mouse Buttons In Game under Mouse buttons to stop that.
When it is on, a press is kept from the game only when all of these hold:
- the button is Mouse 3, 4 or 5 and it is bound to an ePOE hotkey, with that hotkey's modifiers held;
- the Path of Exile 2 window is focused.
Your ePOE hotkey still fires. Other apps, unbound buttons, and the left and right buttons are never touched.
Safety
- It is off by default and stays on the machine where you turned it on — Cloud Sync never copies it.
- It runs as a small separate helper. If the helper cannot start, crashes or is stopped, clicks simply reach the game again — it fails open, never with a dead mouse.
- If the game runs as administrator and ePOE does not, Windows will not let the helper see the game's input, so nothing is blocked.
- Some security software flags scripts that install input hooks. If it is blocked, clicks reach the game as before.
